Say “I’ll refer her to endocrinology” during a consult and, once the note is enhanced, a suggestion chip appears under the note tabs. Tap it and Scribe0 writes the referral letter into a new version tab, framed with your clinician profile and letterhead.

Nothing is ever generated without a tap. A suggestion you ignore costs nothing; a suggestion you confirm becomes a document. That asymmetry is deliberate: the clinician stays the author of record.

Detection also understands the difference between a referral you are making and one you are only talking about. “I referred her last year”, “the GP already referred him”, and “if her eGFR drops I’ll refer” all produce no chip; only a commitment you are making now does.

Detection runs on your Mac, for everyone, using the same on-device model as local drafting. It costs nothing and no transcript leaves your machine to produce a suggestion. Writing the letter itself is a Pro feature and runs on the sovereign cloud route.

Settings → Actions lets you edit what Scribe0 looks for and how it writes each document; adding an action like “medical certificate” is a matter of describing it, not writing code. Scribe0 always applies its own fidelity rules on top of your wording: it will never alter a dictated dose, date, or result, and never invent one.
